The Integrated Clinical and Social Systems for the Prevention and Management of Obesity Innovation Collaborative is an ad hoc activity associated with the Roundtable on Obesity Solutions at the National Academies of Sciences, Engineering, and Medicine (The National Academies). This ad hoc convening activity fosters information sharing and collaboration toward the Roundtable aims as well as progress on findings highlighted in prior National Academies reports.

Background
The present system of clinical care is well suited for acute diseases but poorly suited for the prevention and control of chronic diseases such as obesity that account for much of our population’s poor health. The current emphasis on the biologic basis of disease has focused care on the consequences of obesity but has failed to address the multiple determinants of obesity within the social and physical environment. A reduction in the prevalence of obesity will require a combination of effective clinical services to help prevent obesity, attain a healthy weight, and to sustain weight loss; and policy, systems, and environmental changes to support and enhance healthy lifestyle changes. Moreover, investments in improved care are likely to have an immediate impact on the substantial costs associated with severe obesity. Improved care will require changes in provider education, attitudes, and practices, as well as appropriate incentives to deliver efficacious care. The prevalence and complexity of obesity require changes in care delivery, including the engagement of interdisciplinary treatment teams, potentially including but not necessarily limited to physicians, nurses, dietician nutritionists, mid-level practitioners, health coaches, and behavioral health professionals.

People engage in a healthcare system integrated within their community, where settings and resources reinforce healthy behaviors and provide needed care. Stakeholders recognize their interdependency and act in a coordinated and collaborative fashion to improve health.
