This workshop of the Forum on Aging, Disability, and Independence explored the role of home health care in supporting aging in place for older adults and individuals with disabilities. Workshop participants discussed innovative models of community-based home health care, as well as the necessary policy reforms and research priorities to clarify and leverage the value of home health care to achieve the Triple Aim of healthcare: improving patient experience, improving health, and reducing costs.
