Integrating Health Care and Social Services for People with Serious Illness: A Workshop

In an effort to better understand and facilitate discussions about the challenges and opportunities related to integrating health care and social services for people with serious illness, the Roundtable on Quality Care for People with Serious Illness held a full-day public workshop, which featured a broad range of experts and stakeholders including researchers, policy analysts, patient and family caregiving advocates, and representatives of federal agencies.

Description

An ad hoc committee will plan and host a one-day workshop to examine issues related to the integration of health care and social services for people with serious illness. The workshop will feature invited presentations and panel discussions on topics that may include:

  • the range of health care and social services (e.g., medical/dental, psychosocial/spiritual, nutrition, transportation, housing, home care, caregiver assessment and training) necessary to improve care for those with serious illness
  • the strengths, limitations, and impact of existing models of integrated services delivery (e.g., Programs of All-Inclusive Care for the Elderly [PACE], Special Needs Plans [SNPs]), the Bridge Model, and novel, innovative approaches to community-based integrated care programs
  • the gaps in and barriers to the delivery of integrated services
  • potential use of blended/braided financing approaches
  • the role of family caregivers in providing social supports and services, and caregiver needs (e.g., emotional, financial, training, respite care)
  • gaps in research and evidence to support potential policy changes to advance the integration of health care and social services delivery

The committee will develop the agenda for the workshop, select speakers and discussants, and moderate the discussions. A proceedings of the presentations and discussions at the workshop will be prepared by a designated rapporteur in accordance with institutional guidelines.
