The Agency for Healthcare Research and Quality (AHRQ), in consultation with the Department of Health and Human Services, requested the National Academies to review a report on effective strategies for reducing medical errors and increasing patient safety. The review is required by section 922(j)(1) of the Patient Safety and Quality Improvement Act of 2005. The Health and Medicine Division will convene an ad hoc committee to conduct a review of this draft report guided by the questions outlined in the scope of work.
