The Iowa D O T storm water oversight quality assurance inspection form is depicted, containing sections for inspection date and time, D N R authorization, project and county details, prime contractor name, inspection made by, and date of previous Q A inspection. Spaces are provided for listing contractor staff and D O T staff, including erosion control technicians with fields for certification and expiration dates, as well as E S C basics training status. There are documentation review checkboxes for the erosion control implementation plan, E C I P updates, and subcontractor co-permittee statements, followed by areas for comments. Inspection reports review includes checkboxes for missing reports and lacking information, along with a space for comments. Next is a section labeled Deficiencies found that lists ten items: silt fencing, ditch checks, sediment control, rock, silt basin, seeding and mulch, housekeeping, discharge locations, and two spaces for other items. Each deficiency item has checkboxes for whether corrective action is required, and a space for comments, recommended improvements, and location.
