New York State DOT SPDES Stormwater Inspection Report Form

The S P D E S stormwater inspection report form is shown, used for contracts under the S P D E S general permit for stormwater discharges from construction activity. The form begins with areas to enter the job stamp, date, day of week, sheet number, time of day, weather, temperature in degrees Fahrenheit, and soil condition. There are checkboxes to indicate the reason for inspection, which include a seven-calendar day inspection, a thirty-day inspection for temporary shutdown, or a subsequent seven-day inspection due to soil disturbance over five acres or at a T M D L or three zero three d watershed site. The form lists codes for erosion and sediment control measures and stormwater management practices to be inspected, such as mulch, seed and mulch, check dams, sediment filter logs, silt fence, sediment trap, turbidity curtains, pipe slope drains, and several others, each assigned a number. The main inspection table includes columns for item identification, location of practice, code number, whether the practice is temporary or permanent, and remarks that describe specific maintenance required, including sediment removal, replacement, or reinstallation. There are ten rows on the first page and five more on the reverse for additional items. Inspectors are instructed to list only practices needing repair, maintenance, or replacement, and to attach color photographs showing both the identified deficiencies and the condition after corrective action. Below the table, a location map is required showing disturbed and stabilized areas since the last inspection. There is a section to identify all locations where stormwater is discharged from the site to a water of the United States or at the construction limits, along with a table for site runoff outlet, type of outlet, receiving water, and description of the runoff and receiving water condition. The number of acres currently disturbed must be entered, and if more than five acres are disturbed, a check must be made whether N Y S D E C was advised. Additional space is provided to describe deficiencies in the S W P P P, with instructions to specify by inspection item number. There is a question about whether significant deficiencies require S W P P P revision, and if so, a revision form must be completed and filed. Spaces are provided for the qualified inspector’s name, consultant name, and signature, with lines for preparer, reviewer, and those receiving copies, such as the engineer-in-charge and resident engineer. The bottom of the form includes a reference to M U R K 6.1 for the report and continuation attached, and a button to print the form.
